When you book online for the first time and set up your client profile, you will be asked to answer a list of health related questions. This information helps me determine what type of massage is appropriate for your individual needs. This informatiom will be stored in a HIPPA compliant system which can be accessed as needed for your session.  If you choose, you can opt to print the hard copy form below instead, and bring this to your session.
